Radiopharmaceutical Therapy for Advanced Prostate Cancer: Advancing Precision Oncology Through Theranostics

Understanding Radiopharmaceuticals
Radiopharmaceuticals combine a targeting molecule with a radioactive isotope to selectively bind to cancer cells and deliver radiation directly to sites of disease. Unlike external beam radiation therapy, which targets a defined anatomical area, radiopharmaceuticals circulate systemically and can identify and treat metastatic lesions throughout the body.
In prostate cancer, many radiopharmaceutical strategies target prostate-specific membrane antigen (PSMA), a transmembrane protein that is highly expressed on the surface of prostate cancer cells, particularly in advanced and metastatic disease.
This targeted mechanism allows clinicians to both visualize disease burden and deliver therapeutic radiation using related compounds directed at the same molecular target.

PSMA PET Imaging
Radiopharmaceutical imaging agents such as Pylarify® (piflufolastat F 18) and Illuccix® (gallium Ga 68 gozetotide) bind to PSMA-expressing cells and allow highly sensitive detection of disease using PET/CT imaging.
Compared with conventional imaging modalities, PSMA PET has demonstrated superior sensitivity for identifying metastatic and recurrent disease, helping clinicians more accurately stage patients, assess disease burden, and guide treatment decisions.

PSMA-Targeted Radioligand Therapy
Building on the same targeting principle, therapeutic radiopharmaceuticals deliver cytotoxic radiation directly to PSMA-positive tumor cells.
Pluvicto® (lutetium Lu 177 vipivotide tetraxetan) is a PSMA-directed radioligand therapy approved for eligible patients with PSMA-positive metastatic castration-resistant prostate cancer. Following intravenous administration, the radioligand binds to PSMA-expressing cells and delivers beta-particle radiation directly to tumor sites throughout the body, including both osseous and soft-tissue metastases.
This targeted delivery mechanism allows treatment of disseminated disease while limiting radiation exposure to non-target tissues.
Patient Selection and Multidisciplinary Evaluation
Appropriate patient selection remains critical to optimizing outcomes with radiopharmaceutical therapy.
Patients considered for PSMA-targeted radioligand therapy typically undergo comprehensive evaluation that includes:
